  1. Provide compassionate and personalized care to patients and their families during their end-of-life journey.
  2. Assist patients with personal care tasks such as bathing, dressing, and grooming.
  3. Monitor and report any changes in patients' physical and emotional well-being to the hospice team.
  4. Create a safe and comfortable environment for patients and their families.
  5. Communicate effectively with patients and their families, as well as the hospice team, to ensure a coordinated and holistic approach to care.
  6. Respect and maintain patient confidentiality at all times.
  7. Adhere to all regulatory and company policies and procedures.
  8. Participate in interdisciplinary team meetings and contribute to care planning.
  9. Provide emotional and spiritual support to patients and their families.
  10. Maintain accurate and timely documentation of patient care activities.
  11. Collaborate with other healthcare professionals to provide comprehensive and coordinated care.
  12. Continuously improve and maintain knowledge and skills through participation in ongoing training and development opportunities.
  13. Maintain a positive and professional attitude towards patients and their families, colleagues, and the hospice organization.
  14. Uphold the values and mission of the hospice organization.
  15. Treat all patients and families with empathy, dignity, and respect.

Job Qualifications
  • Certified Nursing Assistant (Cna) Certification Or Equivalent Training And Experience In Providing Personal Care Services To Patients.

  • Minimum Of 1 Year Of Experience In A Healthcare Setting, Preferably In Hospice Or Home Health Care.

  • Knowledge And Understanding Of Hospice Philosophy, Principles, And Practices.

  • Ability To Communicate Effectively And Empathetically With Patients, Families, And Interdisciplinary Team Members.

  • Physical Ability To Lift, Transfer, And Provide Personal Care To Patients With Limited Mobility.
