- Conduct comprehensive assessments of patient's health and medical history to develop individualized care plans.
- Utilize knowledge and understanding of OASIS assessments to accurately document and report patient's condition and progress.
- Educate patients and their families on home healthcare services and the care plan developed.
- Monitor and evaluate patient's response to treatment and make necessary adjustments to the care plan as needed.
- Collaborate and communicate effectively with interdisciplinary team members to ensure coordinated and quality care for each patient.
- Adhere to all regulatory guidelines and standards for home healthcare services.
- Maintain accurate and timely documentation of all patient interactions and interventions.
- Provide compassionate and empathetic care while maintaining professionalism and confidentiality.
- Continuously enhance knowledge and skills through ongoing education and training opportunities.
- Promote a positive and welcoming atmosphere for patients and their families, as well as fellow team members.
- Adhere to company policies and procedures to ensure the highest level of patient care and safety.

Active Rn License: A Qualified Candidate For A Home Health Oasis Rn Job At Amedisys Must Hold An Active And Unrestricted Rn License In The State Where They Will Be Practicing.
Oasis Certification: A Candidate Must Have Completed A Comprehensive Training Program In Outcome And Assessment Information Set (Oasis) And Be Certified In Oasis Assessment.
Minimum Of 1 Year Of Home Health Experience: Amedisys Prefers Candidates With At Least One Year Of Experience In A Home Health Setting, As They Will Be Responsible For Conducting In-Home Patient Assessments.
Strong Clinical Skills: The Ideal Candidate Should Possess Strong Clinical Skills And Knowledge In Wound Care, Medication Management, And Disease Management To Provide High-Quality Care To Patients.
Excellent Communication And Documentation Skills: As A Home Health Oasis Rn, The Candidate Must Have Excellent Communication And Documentation Skills To Accurately Document Patient Information And Coordinate Care With Other Healthcare Team Members.
