1. Provide compassionate and personalized care to patients in their homes.
2. Assist patients with daily tasks such as bathing, grooming, and dressing.
3. Monitor and record patients' vital signs and report any changes or concerns to the appropriate healthcare professionals.
4. Administer medication as directed by the patient's healthcare team.
5. Help patients with mobility and ensure their safety at all times.
6. Communicate effectively with patients and their families, providing emotional support and addressing any questions or concerns.
7. Maintain accurate and detailed records of patient care, including progress notes and observations.
8. Follow all infection control protocols and maintain a clean and safe environment for patients.
9. Collaborate with other healthcare professionals to develop and implement personalized care plans for patients.
10. Attend training and educational sessions to continuously improve skills and knowledge as a Home Health Aide.
11. Adhere to all company policies and procedures to ensure high-quality patient care.
12. Maintain confidentiality of patient information and adhere to HIPAA regulations.
13. Demonstrate a positive and professional attitude in all interactions with patients, families, and coworkers.
14. Participate in team meetings and contribute to the overall success of the home health care team.
15. Perform other duties as assigned by the supervisor or manager.
