CNA Hospice Aide

1. Provide compassionate and personalized care to hospice patients and their families.
2. Assist patients with daily living activities, such as bathing, dressing, and grooming.
3. Monitor and report any changes in patients' physical or emotional conditions to the hospice team.
4. Create a comfortable and supportive environment for patients and their families.
5. Administer medications and treatments as directed by the hospice team.
6. Document patient care accurately and timely in accordance with company policies and procedures.
7. Collaborate with the hospice team to develop and implement individualized care plans for patients.
8. Communicate effectively with patients, families, and interdisciplinary team members.
9. Provide emotional support and companionship to patients and their families during end-of-life journeys.
10. Adhere to all safety, infection control, and HIPAA regulations.
11. Maintain a professional and empathetic attitude towards patients and their families.
12. Attend educational and training programs to stay updated on hospice care best practices.
13. Participate in quality improvement initiatives to enhance patient care.
14. Display a high level of integrity and confidentiality in all aspects of the job.
15. Perform other duties as assigned by the hospice team.
