  1. Oversee and coordinate patient care services to ensure high quality and compassionate care for all patients.
  2. Lead and manage a team of healthcare professionals, including nurses and therapists, to ensure efficient and effective delivery of care.
  3. Monitor and evaluate patient outcomes and develop strategies to improve care quality.
  4. Develop and implement policies and procedures to ensure compliance with all healthcare regulations and standards.
  5. Collaborate with other healthcare providers, including physicians and social workers, to coordinate and enhance patient care.
  6. Provide guidance and support to staff, including training, mentoring, and performance evaluations.
  7. Ensure accurate and timely documentation of patient care and maintain confidentiality of patient information.
  8. Participate in interdisciplinary team meetings and make recommendations for patient care plans.
  9. Stay updated on current healthcare trends and best practices, and integrate them into the organization's operations.
  10. Foster a positive and collaborative work environment that promotes teamwork, communication, and professional growth.
  11. Handle any patient or staff complaints or concerns in a timely and professional manner.
  12. Represent the organization in a professional and ethical manner at all times.
  13. Comply with all company policies, procedures, and protocols.
  14. Participate in budget planning and management to ensure efficient use of resources.
  15. Continuously evaluate and improve processes to enhance patient care and operational efficiency.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Nursing, Or A Related Field

  • Minimum Of 5 Years Experience In A Clinical Leadership Role, Preferably In A Home Healthcare Or Hospice Setting

  • Strong Knowledge Of Healthcare Regulations And Compliance Standards

  • Excellent Communication And Interpersonal Skills, With The Ability To Effectively Lead And Manage A Team

  • Current State Licensure As A Registered Nurse (Rn) Or Other Relevant Healthcare License/Certification
