CHHA Hospice Aide

1. Provide compassionate and personalized care to hospice patients in their homes.
2. Assist patients with activities of daily living, including bathing, dressing, and grooming.
3. Monitor and report any changes in patient condition or behavior to the hospice team.
4. Administer medication and assist with medication management under the direction of a registered nurse.
5. Provide emotional support and companionship to patients and their families.
6. Maintain a safe and comfortable environment for patients, following all infection control and safety protocols.
7. Complete all necessary documentation accurately and in a timely manner.
8. Collaborate with the hospice interdisciplinary team to develop and implement individualized care plans.
9. Participate in ongoing education and training to maintain and improve skills and knowledge.
10. Promote the mission and values of Amedisys and exhibit professionalism and empathy in all interactions with patients, families, and colleagues.
