- Coordinate and facilitate the transition of patients from hospital to home care services.
- Develop and maintain relationships with hospitals and discharge planners to ensure smooth and efficient transitions.
- Communicate with patients and their families to provide information about the home care services and address any concerns or questions.
- Conduct thorough assessments of patients' needs and develop individualized care plans.
- Collaborate with the interdisciplinary team to ensure proper care coordination and continuity of care.
- Monitor and track patient progress and make necessary adjustments to care plans as needed.
- Educate patients and their families on health conditions, medications, and self-care techniques.
- Ensure compliance with all regulatory and company policies and procedures.
- Utilize excellent communication and organizational skills to effectively manage caseload and maintain accurate documentation.
- Participate in team meetings and provide input on ways to improve patient outcomes and overall care delivery.
- Stay updated on industry trends and best practices to continuously improve care delivery.
- Act as a liaison between patients, caregivers, and other healthcare providers to ensure all parties are informed and involved in the care process.

Valid Rn Or Lpn License: A Qualified Candidate For The Care Transitions Coordinator Rn/Lpn Liaison Job At Amedisys Should Hold A Valid And Active Rn Or Lpn License In The State Where They Will Be Practicing.
Minimum Of 2 Years Clinical Experience: Amedisys Requires Candidates To Have A Minimum Of 2 Years Of Experience Working In A Clinical Setting, Preferably In Home Health, Hospice, Or A Related Field.
Knowledge Of Care Transitions And Discharge Planning: The Ideal Candidate Should Have A Strong Understanding Of Care Transitions And Discharge Planning Processes In Order To Effectively Coordinate And Manage Patient Care Between Different Healthcare Settings.
Excellent Communication And Interpersonal Skills: As A Liaison Between Patients, Families, And Healthcare Providers, The Care Transitions Coordinator Must Possess Strong Communication And Interpersonal Skills To Effectively Collaborate And Advocate For Patients.
Time Management And Organizational Abilities: This Position Requires A High Level Of Organization And The Ability To Manage Multiple Tasks And Deadlines. A Candidate Should Have Strong Time Management Skills And Be Able To Prioritize And Effectively Manage Their Workload.
