
Care Transitions Coordinator

Bachelor's Degree In Nursing, Social Work, Or A Related Field.
Minimum Of 2 Years Of Experience In Care Coordination Or Case Management.
Knowledge Of Medicare And Medicaid Regulations And Guidelines.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Patients, Families, And Healthcare Providers.
Certification In Care Transitions Management (Ctm) Or Willingness To Obtain Certification Within 6 Months Of Employment.
