  1. Develop and maintain relationships with healthcare facilities, providers, and community organizations to facilitate patient transitions between different levels of care.
  2. Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients.
  3. Conduct assessments of patients' needs and coordinate appropriate services to ensure smooth transitions.
  4. Educate patients and their families on available resources and support services.
  5. Monitor patient progress and communicate with healthcare providers to ensure continuity of care.
  6. Adhere to all regulatory guidelines and maintain accurate and up-to-date documentation.
  7. Identify and address any barriers to successful transitions, such as financial or logistical challenges.
  8. Participate in quality improvement initiatives to enhance the overall patient experience.
  9. Maintain a thorough understanding of healthcare systems and stay updated on industry changes and advancements.

Job Qualifications
  • Excellent Communication And Interpersonal Skills

  • Bachelor's Degree In Nursing Or A Related Field

  • Minimum Of 2-3 Years Experience In Care Coordination Or Case Management

  • Strong Knowledge Of Healthcare Systems, Including Medicare And Medicaid
