1. Coordinate and manage the care transition process for patients moving from hospital to home.
2. Communicate effectively with patients, their families, and healthcare professionals to ensure a smooth transition.
3. Conduct assessments to determine the needs and preferences of patients and develop individualized care plans.
4. Collaborate with interdisciplinary teams to ensure coordinated and comprehensive care for patients.
