  1. Coordinate patient transitions from acute care to home health services.
  2. Communicate with hospital discharge planners and other healthcare providers to ensure a smooth transition for patients.
  3. Collaborate with interdisciplinary team members to develop and implement individualized care plans for patients.
  4. Educate patients and their families on the home health services available and answer any questions or concerns.
  5. Conduct assessments of patients' needs and provide appropriate referrals to other healthcare resources.
  6. Maintain accurate and up-to-date patient records and documentation.
  7. Advocate for patients' needs and ensure their preferences are considered in their care plan.
  8. Monitor patient progress and communicate any changes or concerns to the appropriate healthcare professionals.
  9. Serve as a liaison between patients, families, and healthcare providers to ensure continuity of care.
  10. Participate in team meetings and provide input on patient care and best practices.
  11. Stay current on industry trends and regulations related to home health services.
  12. Provide exceptional customer service and maintain a positive and professional attitude towards patients, families, and coworkers.

Job Qualifications
  • Bachelor's Degree In Nursing, Social Work, Or Healthcare Administration.

  • Minimum Of 2 Years Experience In Care Coordination Or Case Management.

  • Knowledge Of Healthcare Regulations And Reimbursement Processes.

  • Excellent Communication And Interpersonal Skills.

  • Ability To Work Independently And Manage A Caseload Of Patients.
