  1. Facilitate smooth transitions for patients from hospital to home by coordinating and managing all aspects of the process.
  2. Conduct thorough assessments of patients' needs and develop personalized care plans in collaboration with healthcare professionals.
  3. Educate patients and their families on available resources and support systems to ensure a successful transition back to their homes.
  4. Communicate effectively with hospital staff, home health teams, and other providers to ensure continuity of care.
  5. Monitor and track patients' progress and provide timely updates to all involved parties.
  6. Identify and address potential barriers to a successful transition, such as financial limitations or lack of caregiver support.
  7. Coordinate necessary medical equipment and supplies for patients' home care needs.
  8. Serve as a liaison between patients, families, and healthcare providers to address any concerns or issues that may arise during the transition process.
  9. Stay up-to-date on industry trends and regulations related to care transitions, and make recommendations for process improvements.
  10. Maintain accurate and thorough documentation of all patient interactions and transition processes.
  11. Uphold the mission and values of Amedisys by providing compassionate and high-quality care to patients and their families.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Nursing, Or A Related Field.

  • Minimum Of 2 Years Experience In Care Coordination Or Case Management, Preferably In A Home Health Or Hospice Setting.

  • Strong Knowledge Of Medicare And Other Third-Party Payers' Regulations And Guidelines.

  • Excellent Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Patients, Families, And Healthcare Providers.

  • Proficient In Electronic Medical Record Systems And Other Relevant Software.
