  1. Act as the primary point of contact for members transitioning from one level of care to another.
  2. Communicate with members, healthcare providers, and other stakeholders to ensure a smooth transition and continuity of care.
  3. Conduct thorough assessments of members' health needs and develop personalized care plans.
  4. Collaborate with interdisciplinary teams to identify and address any barriers to successful transition.
  5. Monitor and track members' progress during and after the transition period.
  6. Provide education and support to members and their families regarding their care plans and available resources.
  7. Maintain accurate and detailed records of all interactions and interventions.
  8. Identify and escalate any potential issues or concerns to the appropriate parties.
  9. Stay up-to-date on industry trends and best practices related to transition of care.
  10. Adhere to all company policies, procedures, and regulatory requirements.
  11. Participate in team meetings and contribute to the development and improvement of processes and protocols.
  12. Provide exceptional customer service to members and stakeholders.
  13. Serve as a compassionate and empathetic advocate for members.
  14. Collaborate with community resources and organizations to ensure members have access to necessary resources.
  15. Maintain confidentiality and handle sensitive information with discretion.

Job Qualifications
  • Bachelor's Degree In Social Work, Nursing, Or A Related Field

  • Minimum Of 2 Years Experience In Healthcare Coordination Or Case Management

  • Knowledge Of Local Healthcare Resources And Community Organizations In Tarrant County, Tx

  • Experience Working With Diverse Populations And Understanding Of Cultural Competency

  • Strong Communication And Interpersonal Skills, With The Ability To Collaborate With A Multidisciplinary Team And Effectively Advocate For Patients' Needs.
