  1. Coordinate and facilitate the transition of patients from hospitals or other care facilities to their homes.
  2. Ensure that patients receive proper care and support during the transition process.
  3. Build strong relationships with patients, healthcare providers, and other stakeholders involved in the transition of care process.
  4. Communicate effectively with patients and their families to provide information and support throughout the transition process.
  5. Collaborate with healthcare providers to develop comprehensive care plans and ensure continuity of care.
  6. Monitor and track patient progress to identify any potential issues and address them promptly.
  7. Advocate for patients and their needs during the transition process.
  8. Conduct assessments to identify individual patient needs and develop personalized care plans.
  9. Coordinate and schedule necessary medical appointments and follow-up care for patients.
  10. Educate patients and their families on their healthcare options and resources available to them.
  11. Stay up-to-date on healthcare policies and regulations to ensure compliance and provide accurate information to patients.
  12. Participate in team meetings and collaborate with colleagues to improve processes and enhance patient care.
  13. Maintain accurate and detailed documentation of patient interactions and progress.
  14. Continuously evaluate and improve the transition of care process to promote positive health outcomes for patients.
  15. Provide exceptional customer service and support to patients and their families.

Job Qualifications
  • Bachelor's Degree In Nursing, Social Work, Or A Related Field.

  • Minimum Of 3 Years Of Experience In Care Coordination, Case Management, Or Transitional Care.

  • Strong Understanding Of Healthcare Systems And Knowledge Of Community Resources.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams.

  • Familiarity With Electronic Medical Records And Ability To Navigate And Document In Various Systems.
