  1. Coordinate and facilitate the transition of care process for Aetna members, ensuring a smooth and efficient transfer from one care setting to another.
  2. Utilize clinical expertise and knowledge of care coordination to assess and identify the needs of members during the transition process.
  3. Collaborate with healthcare providers, facilities, and other members of the healthcare team to develop and implement individualized care plans for members.
  4. Conduct thorough assessments and reviews of medical records to ensure accurate and timely transfer of information.
  5. Communicate effectively with members, families, and caregivers to educate and support them during the transition process.
  6. Monitor and track member progress during the transition of care, identifying and addressing any barriers or challenges.
  7. Work closely with other Aetna departments to ensure a seamless and coordinated transition of care for members.
  8. Adhere to all regulatory and compliance guidelines, including HIPAA and state/federal regulations.
  9. Maintain accurate and thorough documentation of all member interactions and care coordination activities.
  10. Continuously evaluate and improve the transition of care process to ensure high-quality and efficient services for Aetna members.
  11. Attend meetings, training sessions, and other educational opportunities to enhance clinical knowledge and skills.
  12. Collaborate with other LVN/LPNs and healthcare professionals to share best practices and promote a culture of continuous learning and improvement.
  13. Demonstrate compassion, empathy, and professionalism in all interactions with members and colleagues.
  14. Comply with all company policies and procedures, including those related to safety, confidentiality, and ethical standards.
  15. Represent Aetna in a positive and professional manner at all times, maintaining the company's reputation for excellence and high-quality care.

Job Qualifications
  • Active Lvn/Lpn License: The Candidate Must Possess A Current And Valid Lvn/Lpn License In The State Where They Will Be Providing Care.

  • Clinical Experience: A Minimum Of 2-3 Years Of Experience As An Lvn/Lpn In A Clinical Setting, Preferably In A Transition Of Care Role, Is Required.

  • Knowledge Of Health Insurance: The Candidate Should Have A Thorough Understanding Of Health Insurance Policies, Claims Processing, And Reimbursement Procedures.

  • Strong Communication Skills: Excellent Verbal And Written Communication Skills Are Essential For This Role, As The Lvn/Lpn Will Be Responsible For Coordinating Care Between Patients, Healthcare Providers, And Insurance Companies.

  • Familiarity With Electronic Medical Records: Proficiency In Using Electronic Medical Records And Other Healthcare Software Is Highly Preferred, As The Lvn/Lpn Will Be Responsible For Documenting Patient Information And Ensuring Accuracy Of Records.
