Job Qualifications
  • Active Lpn License In New Jersey And Pennsylvania: A Transition Of Care Associate Lpn Must Possess A Valid And Active Lpn License In Both New Jersey And Pennsylvania, As The Job Involves Working In The Field And Providing Care To Patients In These States.

  • Minimum Of 2 Years Of Clinical Experience: Aetna Requires Candidates For This Position To Have A Minimum Of 2 Years Of Clinical Experience, Preferably In A Care Coordination Or Transitional Care Setting. This Experience Ensures That The Candidate Has A Strong Understanding Of Patient Care And Can Effectively Transition Patients From One Setting To Another.

  • Knowledge Of Medicare And Medicaid Regulations: As A Transition Of Care Associate, It Is Important To Have A Thorough Understanding Of Medicare And Medicaid Regulations, As Many Patients In This Role Will Be Covered By These Government Programs. Knowledge Of These Regulations Will Ensure That Patients Receive Appropriate Care And Services.

  • Strong Communication And Interpersonal Skills: This Role Requires Frequent Communication With Patients, Caregivers, And Healthcare Providers, So It Is Essential For A Transition Of Care Associate Lpn To Have Strong Communication And Interpersonal Skills. This Includes The Ability To Listen Actively, Convey Information Effectively, And Build Rapport With Patients And Colleagues.

  • Ability To Work Independently And In A Team: Aetna Values Candidates Who Can Work Independently And Take Initiative, As Well As Collaborate Effectively As Part Of A Team.
