  1. Coordinate care for members transitioning from one level of care to another, ensuring a smooth and seamless experience.
  2. Serve as a primary point of contact for members, healthcare providers, and other stakeholders involved in the transition process.
  3. Communicate and collaborate with various departments within Aetna to gather necessary information and resources for members.
  4. Assess members' needs and develop individualized transition plans to meet those needs.
  5. Monitor and track members' progress throughout the transition process, ensuring timely and appropriate care is received.
  6. Educate members and caregivers on available resources and support services to aid in the transition process.
  7. Advocate for members' needs and address any barriers or challenges that may arise during the transition.
  8. Document and maintain accurate records of all interactions and activities related to the transition of care.
  9. Stay up-to-date on healthcare policies, procedures, and regulations to ensure compliance and appropriate care for members.
  10. Collaborate with healthcare providers and community resources to ensure a comprehensive and coordinated transition of care for members.
  11. Provide support and guidance to members and caregivers during the transition process, including addressing any questions or concerns.
  12. Participate in quality improvement initiatives to enhance the transition of care process and improve member outcomes.
  13. Serve as a compassionate and empathetic representative of Aetna, embodying the company's values and commitment to improving the healthcare experience for members.

Job Qualifications
  • Bachelor's Degree In Healthcare Administration, Nursing, Or A Related Field.

  • Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In Care Coordination Or Care Management.

  • Strong Knowledge Of Healthcare Systems And Regulations, Including Medicare And Medicaid.

  • Excellent Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Various Healthcare Professionals.

  • Proficiency In Electronic Health Record Systems And Other Relevant Technology Platforms.
