6. Advocate for members' rights and needs, including ensuring access to necessary healthcare services and addressing any barriers to care.
7. Maintain accurate and timely documentation of member interactions and progress towards goals.
8. Participate in team meetings and trainings to stay updated on healthcare policies and procedures.
9. Utilize evidence-based practices and cultural competency to provide high-quality and equitable care to all members.
10. Continuously evaluate and improve social work processes and procedures to enhance member experience and outcomes.
