- Develop and implement population health strategies and initiatives to improve health outcomes for Aetna members
- Collaborate with cross-functional teams to identify and address population health needs and gaps in care
- Analyze population health data and trends to identify opportunities for improvement and measure the impact of initiatives
- Provide leadership and guidance to a team of healthcare professionals, ensuring the successful execution of population health programs
- Foster relationships with key stakeholders, including healthcare providers, community organizations, and government agencies, to support population health efforts
- Monitor and ensure compliance with regulatory and accreditation requirements related to population health
- Develop and manage budgets for population health initiatives
- Stay current on industry trends and best practices in population health management
- Communicate effectively with team members, colleagues, and leadership to provide updates on population health initiatives and outcomes
- Represent Aetna at conferences, events, and meetings related to population health management.
A Bachelor's Degree In Public Health, Healthcare Administration, Or A Related Field.
At Least 3-5 Years Of Experience In Population Health Management, Preferably In A Healthcare Or Insurance Setting.
Knowledge Of Healthcare Policies, Regulations, And Best Practices Related To Population Health And Value-Based Care.
Strong Analytical Skills And Experience With Data Analysis And Reporting, Including Experience With Population Health Management Software.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Various Stakeholders, Including Providers, Payers, And Community Organizations.
