- Develop and implement population health management strategies to improve health outcomes and reduce healthcare costs
- Collaborate with internal and external stakeholders, such as healthcare providers, community organizations, and government agencies, to identify and address population health needs
- Analyze and interpret data to identify trends and opportunities for intervention in population health
- Lead and manage cross-functional teams to execute population health initiatives and drive performance
- Stay informed on industry trends and best practices in population health management and incorporate them into Aetna's strategies
- Serve as a subject matter expert on population health management and provide guidance and support to other team members
- Monitor and evaluate the effectiveness of population health programs and make recommendations for improvements
- Develop and manage budgets and resources for population health projects
- Communicate and present population health initiatives and outcomes to various audiences, including senior leadership and external partners
- Foster a culture of innovation and continuous improvement within the population health team
- Represent Aetna in external forums and conferences to promote the company's population health efforts and establish partnerships.
Bachelor's Degree In Public Health, Healthcare Administration, Or A Related Field.
Minimum Of 3-5 Years Of Experience In Population Health Management Or A Similar Role In The Healthcare Industry.
Knowledge Of Healthcare Policies, Regulations, And Quality Improvement Initiatives.
Strong Analytical Skills And Experience With Data Analysis And Reporting.
Excellent Communication And Leadership Skills, With The Ability To Collaborate With Various Stakeholders And Drive Change Within The Organization.
