  1. Develop and implement strategies to improve population health outcomes for diverse member population.
  2. Collaborate with cross-functional teams to identify and address gaps in current population health management processes.
  3. Utilize data analysis and reporting to monitor and evaluate the effectiveness of population health programs.
  4. Lead and manage a team of healthcare professionals to ensure the delivery of high-quality population health services.
  5. Stay updated on industry trends and best practices in population health management to continuously improve processes and outcomes.
  6. Develop and maintain relationships with external partners and stakeholders to support population health initiatives.
  7. Create and maintain a positive and inclusive work environment that promotes teamwork and collaboration.
  8. Communicate effectively with various stakeholders, including senior leadership, to provide updates on population health initiatives and progress.
  9. Develop and manage budgets for population health programs and initiatives.
  10. Identify and implement innovative solutions to address health disparities and improve overall community health.
  11. Ensure compliance with all relevant laws, regulations, and policies related to population health management.
  12. Continuously evaluate and improve processes to enhance the efficiency and effectiveness of population health programs.
  13. Serve as a subject matter expert and resource for population health management within the organization.
  14. Participate in conferences, seminars, and other professional development opportunities to stay current on population health trends and best practices.
  15. Act as a role model for the organization by promoting a culture of health and wellness.

Job Qualifications
  • Bachelor's Or Master's Degree In Public Health, Epidemiology, Healthcare Administration, Or A Related Field.

  • Minimum Of 5 Years Experience In A Healthcare Or Public Health Setting, With A Focus On Population Health Management.

  • Strong Knowledge Of Healthcare Systems, Policies, And Regulations, Including Medicare And Medicaid.

  • Demonstrated Experience In Developing, Implementing, And Evaluating Population Health Programs And Initiatives.

  • Excellent Communication, Leadership, And Project Management Skills, With The Ability To Collaborate With Diverse Stakeholders And Manage A Team.
