  1. Provide high-quality and compassionate case management services to individuals and families in need of support.
  2. Collaborate with a diverse team to develop and implement effective care plans for each member.
  3. Conduct thorough assessments to identify the needs and goals of each member, and regularly reassess to ensure their needs are being met.
  4. Utilize a strength-based approach to empower members and their families to take an active role in their healthcare.
  5. Maintain accurate and detailed case notes, documenting all interactions with members and their families.
  6. Serve as a liaison between members, their families, and other healthcare providers to ensure a coordinated and comprehensive approach to care.
  7. Advocate for members to ensure they receive appropriate and timely services.
  8. Provide education and support to members and their families on managing chronic conditions, navigating the healthcare system, and accessing community resources.
  9. Participate in regular team meetings and case conferences to discuss member progress and develop strategies for addressing challenges.
  10. Stay up-to-date on current healthcare policies and regulations to ensure compliance and provide the best possible care to members.
  11. Adhere to ethical and professional standards in all interactions with members and their families.
  12. Continuously evaluate and improve the effectiveness of the High Fidelity Wraparound program.
  13. Adhere to all Aetna policies and procedures, including maintaining member confidentiality.

Job Qualifications
  • Bachelor's Degree In Social Work, Psychology, Counseling, Or A Related Field.

  • Excellent Communication, Organizational, And Problem-Solving Skills.

  • Minimum Of 2 Years Of Experience In Case Management, Preferably In A Behavioral Health Setting.

  • Strong Understanding Of High Fidelity Wraparound Principles And Experience Implementing Them In A Professional Setting.

  • Knowledge Of Community Resources And Ability To Coordinate Services For Clients.
