• Develop and maintain positive relationships with individuals and families in need of wraparound services
• Conduct thorough assessments of clients' needs and create individualized care plans
• Collaborate with community resources and service providers to connect clients with necessary support and services
• Monitor and evaluate progress of clients' care plans, making adjustments as needed
• Provide crisis intervention and support to clients and their families as needed
• Advocate for clients and their families to ensure their needs are met and their voices are heard
• Maintain accurate and timely documentation of all client interactions and progress
• Attend and actively participate in team meetings and trainings to enhance skills and knowledge
• Keep up-to-date with current best practices and policies related to case management and wraparound services
