1. Provide personalized health coaching to Aetna members via phone or virtual platforms.
2. Empower and motivate members to take control of their health and well-being.
3. Develop and implement individualized health plans based on members' needs and goals.
4. Utilize evidence-based practices and resources to educate members on healthy lifestyle behaviors.
5. Maintain a strong understanding of Aetna's health and wellness programs, policies, and procedures.
6. Collaborate with other health professionals, such as nurses and physicians, to ensure coordinated and comprehensive care for members.
7. Monitor and track members' progress towards health goals and provide ongoing support and guidance.
8. Act as a resource for members, addressing questions and concerns related to their health and wellness.
9. Document all interactions and interventions with members accurately and timely in Aetna's systems.
10. Stay updated on industry trends and advancements in health coaching to continuously improve and enhance services.
11. Adhere to all HIPAA and confidentiality guidelines when handling sensitive member information.
12. Participate in trainings and regular team meetings to enhance skills and knowledge.
13. Provide exceptional customer service and maintain a positive and professional demeanor at all times.
14. Collaborate with team members to develop and implement strategies to improve member engagement and outcomes.
15. Contribute to a positive and inclusive work environment by promoting teamwork, open communication, and a shared commitment to Aetna's mission and values.
