- Coordinate and manage the care plans of Aetna members in the Houston, TX area.
- Provide exceptional care and support to members in accordance with Aetna's standards and policies.
- Promote health and wellness by educating and empowering members to make informed decisions about their health.
- Advocate for members' needs and collaborate with healthcare providers to ensure their medical needs are met.
- Conduct thorough assessments of members' health and identify any potential barriers to care.
- Develop individualized care plans that address members' physical, emotional, and social needs.
- Monitor and evaluate members' progress towards health goals and make necessary adjustments to their care plans.
- Communicate with members, families, and healthcare providers to ensure continuity and coordination of care.
- Keep accurate and up-to-date documentation of member interactions, care plans, and progress.
- Stay current on industry trends and best practices in care management, and apply them to improve the quality of care for members.
- Collaborate with other members of the Aetna team to ensure seamless and efficient delivery of care.
- Maintain confidentiality and adhere to all HIPAA and privacy regulations.
- Act as a resource and support system for members, addressing any concerns or questions they may have.
- Participate in training and development opportunities to continuously enhance skills and knowledge.
- Ensure compliance with all regulatory requirements and company policies.
Valid Rn Licensure In The State Of Texas: The Field Care Manager Must Be A Registered Nurse With An Active License In The State Of Texas, Specifically In The Houston Area (Zip Codes 77004 And 77030).
Minimum Of 3 Years Clinical Experience: Aetna Is Looking For Candidates With At Least 3 Years Of Clinical Experience, Preferably In A Home Health Or Case Management Setting. This Experience Will Provide The Necessary Foundation For Managing Care Plans And Coordinating Services For Members.
Strong Communication And Interpersonal Skills: As A Field Care Manager, The Ability To Effectively Communicate With Members, Providers, And Other Healthcare Professionals Is Crucial. The Ideal Candidate Will Possess Excellent Communication And Interpersonal Skills To Build Relationships And Facilitate Care.
Knowledge Of Medicare And Medicaid Regulations: Aetna Serves A Large Population Of Members Who Are Covered Under Medicare And Medicaid Plans. The Field Care Manager Must Have A Strong Understanding Of These Regulations And Be Able To Navigate Them While Managing Care For Members.
