- Conduct outreach and engage with underserved populations in Marion and Wayne County, IL to assess their health needs and connect them with appropriate resources and services.
- Develop and maintain relationships with community organizations, healthcare providers, and other stakeholders to facilitate the delivery of health education and support services.
- Provide culturally competent care and advocate for the needs of diverse populations, including but not limited to low-income individuals, immigrants, and people with disabilities.
- Collaborate with healthcare professionals to develop and implement care plans for individuals and families, ensuring continuity of care and follow-up.
- Educate individuals and families on disease prevention, healthy behaviors, and self-management techniques.
- Document all interactions and services provided accurately and in a timely manner.
- Conduct home visits and accompany individuals to appointments as needed.
- Participate in community events and health fairs to raise awareness and promote health initiatives.
- Stay up-to-date on local resources and services available to individuals and families, and make appropriate referrals.
- Maintain confidentiality and abide by all ethical and professional standards.
- Attend trainings and meetings as required by Aetna.
- Collaborate with other Community Health Workers and Aetna staff to share best practices and improve health outcomes for the community.
- Comply with all Aetna policies and procedures.
- Other duties as assigned by Aetna.
Certification Or Degree In Community Health Or Related Field: A Community Health Worker At Aetna Should Have Formal Education Or Training In Community Health, Public Health, Or A Related Field. This Will Ensure That They Have A Strong Foundation In The Principles Of Community Health And Are Equipped With The Necessary Skills To Perform Their Job Effectively.
Knowledge Of Community Resources: Community Health Workers Must Have A Thorough Understanding Of The Resources Available In Marion And Wayne County, Il. This Includes Knowledge Of Local Health Care Facilities, Social Service Agencies, And Community Organizations. They Should Also Be Able To Navigate And Access These Resources To Assist Individuals In Need.
