  1. Provide compassionate and empathetic support to individuals facing behavioral health challenges.
  2. Work closely with clients to assess their needs and develop personalized treatment plans.
  3. Conduct thorough assessments to identify mental health needs and develop appropriate interventions.
  4. Collaborate with healthcare professionals to coordinate and monitor client care.
  5. Regularly communicate with clients to ensure their needs are being met and address any concerns or barriers.
  6. Maintain accurate and up-to-date client records and documentation.
  7. Utilize evidence-based practices and treatment modalities to support client recovery.
  8. Advocate for clients and assist with accessing necessary resources and services.
  9. Monitor client progress and make necessary adjustments to treatment plans.
  10. Educate clients and their families on mental health and wellness strategies.
  11. Provide crisis intervention and support as needed.
  12. Stay current on industry trends and best practices in mental health and case management.
  13. Collaborate with team members to ensure a comprehensive and coordinated approach to client care.
  14. Adhere to all ethical and legal standards in client care.
  15. Participate in regular supervision and training to enhance skills and knowledge.
  16. Maintain confidentiality of client information at all times.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Psychology, Or Related Field.

  • Minimum Of 3 Years Experience In Behavioral Health Case Management.

  • Knowledge Of And Experience With Mental Health And Substance Abuse Treatment Modalities.

  • Strong Understanding Of Regulatory And Compliance Requirements For Behavioral Health Services.

  • Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With Multidisciplinary Teams And Effectively Advocate For Members' Needs.
