  1. Conduct comprehensive assessments of members' behavioral health needs and develop individualized case management plans.
  2. Provide ongoing support and advocacy for Spanish-speaking members with mental health conditions, ensuring access to appropriate healthcare services.
  3. Collaborate with healthcare providers and community resources to coordinate care and ensure continuity of services for members.
  4. Monitor and track progress of members, identifying any barriers or challenges to treatment and implementing interventions as needed.
  5. Educate members and their families on available resources and support services, promoting self-management and empowerment.
  6. Maintain accurate and timely documentation of all case management activities and progress.
  7. Adhere to all ethical and legal standards of practice, maintaining confidentiality and upholding the well-being of members.
  8. Stay up-to-date on current trends and best practices in behavioral health and utilize this knowledge to inform and improve case management services.
  9. Provide cultural competency and sensitivity in working with Spanish-speaking members, ensuring equitable and inclusive care.
  10. Collaborate with other members of the healthcare team and participate in interdisciplinary meetings to ensure holistic care for members.
  11. Identify and report any potential quality of care issues and make recommendations for improvement.
  12. Act as a liaison between members, providers, and insurance companies to facilitate efficient and effective care.
  13. Participate in ongoing training and development opportunities to enhance skills and knowledge in behavioral health and case management.
  14. Adhere to all company policies, procedures, and standards to maintain a safe and positive work environment.
  15. Serve as a positive representative of Aetna, promoting our mission and values in all interactions with members and colleagues.

Job Qualifications
  • Fluency In Spanish: The Ability To Communicate Effectively In Spanish Is Essential For This Role, As The Clinical Case Manager Will Be Working With Spanish-Speaking Individuals And Families.

  • Bachelor's Or Master's Degree In Social Work Or Related Field: Aetna Typically Requires Candidates For This Position To Have A Minimum Of A Bachelor's Degree In Social Work Or A Related Field. A Master's Degree May Be Preferred.

  • Experience In Behavioral Health: Candidates Should Have A Strong Background In Behavioral Health, With At Least 2-3 Years Of Experience In The Field. Additional Experience In Case Management Or Care Coordination Is Also Desirable.

  • Knowledge Of Community Resources: The Ideal Candidate Will Have A Strong Understanding Of Community Resources And Support Services For Individuals With Mental Health And Substance Abuse Needs. This Includes Knowledge Of Local Treatment Providers, Support Groups, And Other Resources.

  • Excellent Communication And Interpersonal Skills: As A Clinical Case Manager, Strong Communication And Interpersonal Skills Are Crucial For Building Relationships With Clients, Collaborating With Treatment Providers, And Advocating For Clients' Needs. Candidates Should Also Have Strong Problem-Solving And Organizational Abilities.
