- Implement care plans: Implement care plans by coordinating and monitoring services, advocating for necessary resources, and ensuring timely access to care.

- Collaborate with healthcare providers: Work closely with healthcare providers to ensure coordinated and comprehensive care for individuals with behavioral health needs.
- Monitor progress: Regularly monitor and evaluate the progress of individuals in the care plan, making necessary adjustments as needed.
- Advocate for individuals: Serve as an advocate for individuals with behavioral health needs, ensuring they receive the care and resources they need to achieve optimal health outcomes.
- Maintain accurate records: Maintain accurate and up-to-date documentation of all interactions, services provided, and progress made by individuals in the care plan.
- Stay informed on industry developments: Keep up-to-date with the latest developments and best practices in the behavioral health field to enhance the quality of care provided.
- Promote a positive and collaborative work environment: Foster a positive and collaborative work environment by actively participating in team meetings, providing support to colleagues, and promoting a culture of empathy and compassion.
Bachelor's Or Master's Degree In Social Work, Counseling, Psychology, Or Related Field.
Minimum Of 3-5 Years Experience In Behavioral Health Case Management.
Strong Understanding Of Mental Health And Substance Abuse Treatment Options And Resources.
Knowledge Of Local Community Resources And Support Services For Individuals With Behavioral Health Needs.
Excellent Communication And Interpersonal Skills, With The Ability To Collaborate With A Multidisciplinary Team And Build Rapport With Clients.
