A Bachelor's degree in a related field and a valid clinical license are also required.   1. Develop and implement individualized care plans for members with behavioral health needs.
  2. Collaborate with members, their families, and providers to ensure comprehensive and effective care.
  3. Utilize strong clinical skills to accurately assess members' needs and make appropriate recommendations.
  4. Provide ongoing support and guidance to members throughout their recovery journey.
  5. Advocate for members' needs and ensure they have access to necessary resources and services.
  6. Maintain accurate and up-to-date documentation of all member interactions and progress.
  7. Stay current on industry trends and best practices in behavioral health case management.
  8. Adhere to all regulatory requirements and company policies and procedures.
  9. Collaborate with other members of the healthcare team to coordinate care and ensure continuity of services.
  10. Attend regular team meetings and participate in training and professional development opportunities.
  11. Uphold the values and mission of the organization in all interactions and duties.
  12. Cultivate a positive and supportive relationship with members, promoting trust and rapport.
  13. Continuously evaluate and monitor the effectiveness of care plans and make adjustments as needed.
  14. Provide crisis intervention services as needed.
  15. Demonstrate empathy, compassion, and a non-judgmental attitude towards members.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Counseling, Psychology, Or A Related Field.

  • Minimum Of 3-5 Years Of Experience In Case Management Or Behavioral Health Services, Preferably In A Clinical Setting.

  • Knowledge And Understanding Of Mental Health And Substance Abuse Disorders, Treatment Modalities, And Community Resources.

  • Strong Communication And Interpersonal Skills To Effectively Engage With Clients, Providers, And Other Stakeholders.

  • Current Licensure Or Certification As A Licensed Clinical Social Worker (Lcsw), Licensed Professional Counselor (Lpc), Or Other Relevant Clinical License In The State Of Louisiana.
