  1. Provide comprehensive case management services to members with behavioral health needs.
  2. Develop and implement individualized treatment plans in collaboration with members, providers, and interdisciplinary teams.
  3. Monitor and evaluate the effectiveness of treatment plans and make necessary adjustments.
  4. Conduct assessments and screenings to identify member needs and develop appropriate interventions.
  5. Utilize evidence-based practices and clinical guidelines to ensure quality care for members.
  6. Collaborate and coordinate care with internal and external providers to ensure continuity of care.
  7. Educate members and their families about their behavioral health condition and available resources.
  8. Advocate for members and assist them in navigating the healthcare system.
  9. Monitor and track member progress and provide updates to appropriate parties.
  10. Ensure compliance with all regulatory and accreditation standards.
  11. Maintain accurate and timely documentation of all services provided.
  12. Participate in ongoing training and professional development to stay current on best practices and industry trends.
  13. Demonstrate a commitment to cultural competence and sensitivity in working with diverse populations.
  14. Uphold the ethical and professional standards of the behavioral health field.
  15. Collaborate with other team members and contribute to a positive and productive work environment.
  16. Represent the company in a professional and positive manner at all times.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work, Psychology, Counseling, Or Related Field.

  • Minimum Of 2 Years Experience In Behavioral Health Case Management.

  • Knowledge And Understanding Of Mental Health Diagnoses And Treatment Options.

  • Excellent Communication And Interpersonal Skills To Effectively Collaborate With Clients, Providers, And Other Members Of The Healthcare Team.

  • Strong Organizational And Time Management Skills To Effectively Manage A Caseload And Meet Deadlines.
