- Coordinate and manage the care of Aetna members, ensuring they receive high-quality, appropriate and timely medical care.
- Utilize your strong clinical background to assess and evaluate members' medical needs and develop individualized care plans.
- Collaborate with healthcare providers and other members of the medical team to ensure coordinated and comprehensive care.
- Serve as the primary point of contact for members and their families, providing support, guidance and education throughout their healthcare journey.
- Monitor and track members' progress and outcomes, making necessary adjustments to care plans as needed.
- Advocate for members and help them navigate the healthcare system to access the resources and services they need.
- Maintain accurate and detailed documentation of all interactions and interventions with members.
- Stay up-to-date on industry trends and best practices in case management to continuously improve the quality of care provided.
- Demonstrate compassion, empathy and understanding towards members and their families during difficult and challenging situations.
- Collaborate with other members of the Aetna team to ensure a seamless and positive experience for members.
Active Rn License: The Case Manager Rn Must Possess A Current, Unrestricted Rn License In The State Where They Will Be Practicing. This Demonstrates Their Clinical Knowledge And Ability To Provide Quality Care.
Bachelor's Degree In Nursing: Aetna Requires A Minimum Of A Bachelor's Degree In Nursing For Their Case Managers. This Higher Level Of Education Ensures A Strong Foundation In Nursing Principles And Skills.
Case Management Certification: Aetna Prefers Candidates Who Hold A Case Management Certification, Such As The Certified Case Manager (Ccm) Or Certified Professional In Healthcare Management (Cphm). This Certification Demonstrates A Commitment To The Field And A Specialized Knowledge Of Case Management.
Prior Case Management Experience: Aetna Looks For Candidates With At Least 2-3 Years Of Experience In Case Management, Preferably In A Healthcare Setting. This Shows That The Candidate Has A Solid Understanding Of The Role And Can Handle The Responsibilities Of A Case Manager Rn.
