- Conduct comprehensive assessments of members' healthcare needs and develop individualized care plans.
- Collaborate with healthcare providers and other members of the interdisciplinary team to ensure coordinated and effective care.

Registered Nurse (Rn) Licensure: A Case Manager Rn Must Have An Active And Unrestricted Rn License In The States Of Tarrant County And Dallas County.
Bachelor's Degree In Nursing: Aetna Prefers Candidates With A Bachelor's Degree In Nursing, As It Demonstrates A Strong Foundation In Nursing Principles And Patient Care.
Case Management Certification: Candidates Should Possess A Current Case Management Certification, Such As Certified Case Manager (Ccm) Or Certified Professional In Healthcare Management (Cphm).
Minimum Of 3-5 Years Of Clinical Experience: Aetna Requires Candidates To Have A Minimum Of 3-5 Years Of Clinical Experience Working As An Rn, Preferably In A Case Management Or Care Coordination Role.
Knowledge Of Population Health And Care Coordination: A Case Manager Rn Should Have A Strong Understanding Of Population Health And Care Coordination Principles, With Experience In Managing Complex Cases And Coordinating Care For Patients With Multiple Chronic Conditions.
