- Work collaboratively with other healthcare professionals to ensure the best possible outcomes for members.
- Conduct thorough assessments of members' medical needs and develop personalized care plans.
- Monitor and evaluate members' progress and adjust care plans as needed.

- Maintain accurate and up-to-date documentation of members' care plans and progress.

- Participate in interdisciplinary team meetings to discuss and coordinate care for members.

Bachelor's Degree In Nursing (Bsn) From An Accredited University
Current And Unrestricted Registered Nurse (Rn) License In The State Of Texas
Minimum Of 3 Years Of Clinical Experience As An Rn, Preferably In Case Management Or Care Coordination
Strong Knowledge Of Healthcare Systems, Insurance, And Utilization Management Processes
Excellent Communication, Organizational, And Critical Thinking Skills To Effectively Manage A Caseload And Collaborate With Interdisciplinary Teams.
