- Provide comprehensive care coordination to members with oncology or transplant needs.
- Conduct thorough assessments and develop individualized care plans for each member.

Current Rn Licensure: A Qualified Candidate For A Case Manager Rn- Oncology Or Transplant Job At Aetna Must Possess A Current And Active Rn License In The State Where The Job Is Located.
Oncology Or Transplant Experience: A Minimum Of 2-3 Years Of Experience In The Field Of Oncology Or Transplant Is Required For This Role. This Includes Experience In Managing Patients With Complex Medical Needs And Coordinating Care With Various Healthcare Providers.
Case Management Certification: Aetna Prefers Candidates Who Have Obtained Certification In Case Management, Such As The Certified Case Manager (Ccm) Or Oncology Certified Nurse (Ocn) Credentials. This Demonstrates Knowledge And Expertise In Managing Patient Care And Navigating The Healthcare System.
Strong Communication And Interpersonal Skills: As A Case Manager Rn, The Ability To Communicate Effectively With Patients, Families, And Healthcare Providers Is Essential.
