- Conduct comprehensive and individualized assessments of members' health and psychosocial needs in the field.
- Develop and implement a personalized care plan for each member, based on their specific needs and goals.
- Coordinate and collaborate with other healthcare professionals and community resources to ensure the best possible outcomes for members.
- Monitor and evaluate the effectiveness of the care plan, making adjustments as needed.
- Provide education and support to members and their families on managing their health conditions and navigating the healthcare system.
- Advocate for the needs and rights of members, ensuring they receive appropriate and timely care.
- Maintain accurate and up-to-date documentation of all case management activities.
- Adhere to all relevant regulations and standards for case management and healthcare.
- Participate in interdisciplinary team meetings and contribute to the development of policies and procedures.
- Maintain confidentiality and professionalism in all interactions with members, colleagues, and other stakeholders.
- Act as a liaison between members, healthcare providers, and insurance companies to facilitate the delivery of quality and cost-effective care.
- Continuously seek opportunities for professional development and stay up-to-date on industry trends and best practices in case management.
- Demonstrate a compassionate and empathetic attitude towards members and their families.
- Maintain a positive and collaborative relationship with colleagues and other healthcare professionals.
- Represent Aetna in a professional and positive manner at all times.
Bachelor's Degree In Nursing From An Accredited Institution
Current And Valid Rn License In The State Of Illinois
Minimum Of 3 Years Of Clinical Experience As An Rn, Preferably In A Case Management Or Community Health Setting
Knowledge Of Medicare And Medicaid Regulations And Experience Working With Vulnerable Populations
Excellent Communication And Interpersonal Skills, With The Ability To Effectively Collaborate With Patients, Healthcare Providers, And Community Resources.
