  1. Provide patient-centered care management and support to individuals in the Southwest side of Chicago.
  2. Conduct comprehensive assessments of patients' physical, emotional, and social needs.
  3. Develop and implement personalized care plans based on patients' unique needs and goals.
  4. Collaborate with patients, families, and healthcare providers to ensure effective communication and coordination of care.
  5. Monitor and evaluate patients' progress, making necessary adjustments to care plans as needed.
  6. Advocate for patients' needs and rights, ensuring they receive quality and appropriate healthcare services.
  7. Provide education and resources to patients and their families to promote health and wellness.
  8. Maintain accurate and timely documentation of patient interactions and care plans.
  9. Stay current on healthcare policies, procedures, and regulations to ensure compliance.
  10. Participate in team meetings and collaborate with other healthcare professionals to improve patient outcomes.
  11. Use critical thinking and problem-solving skills to address complex patient situations and make sound decisions.
  12. Demonstrate compassion, empathy, and cultural sensitivity in all interactions with patients and their families.
  13. Serve as a positive representative of Aetna and uphold the company's values and standards.
  14. Act as a mentor to other healthcare professionals, providing guidance and support as needed.

Job Qualifications
  • Valid Rn License In The State Of Illinois And At Least 2 Years Of Clinical Experience In A Healthcare Setting.

  • Bachelor's Degree In Nursing Or Related Field, Or Equivalent Combination Of Education And Experience.

  • Prior Experience In Case Management, Utilization Review, Or Care Coordination, With A Strong Understanding Of Healthcare Delivery Systems And Insurance Regulations.

  • Excellent Communication And Interpersonal Skills, With The Ability To Build Rapport With Diverse Populations And Collaborate With Interdisciplinary Teams.

  • Residency In The Southwest Side Of Chicago, With A Strong Knowledge Of The Local Healthcare Resources And Community Resources Available To Support Patient Care.
