  1. Provide personalized care management to members in their homes, utilizing clinical skills and expertise.
  2. Conduct comprehensive assessments and develop individualized care plans for members.
  3. Collaborate with interdisciplinary teams to coordinate and facilitate high-quality healthcare services.
  4. Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
  5. Educate members and their families on disease management, preventative care, and self-care techniques.
  6. Ensure timely and accurate documentation of all patient interactions and interventions.
  7. Advocate for the needs and rights of patients, while promoting a patient-centered approach to care.
  8. Participate in ongoing training and development to stay updated on industry standards and best practices.
  9. Maintain accurate and up-to-date knowledge of Aetna policies and procedures.
  10. Utilize critical thinking and problem-solving skills to address complex patient situations.
  11. Demonstrate empathy, compassion, and professionalism when communicating with members and their families.
  12. Adhere to all ethical and legal standards of nursing practice.
  13. Comply with all regulatory requirements and maintain patient confidentiality.
  14. Act as a liaison between patients, healthcare providers, and insurance companies to ensure seamless coordination of care.
  15. Continuously strive to improve the quality of care and patient outcomes through continuous quality improvement initiatives.

Job Qualifications
  • Registered Nurse (Rn) License: The Candidate Must Possess A Current And Active Rn License In The State Where The Job Is Located.

  • Case Management Experience: A Minimum Of 3-5 Years Of Experience In Case Management, Preferably In A Field-Based Setting, Is Required For This Role.

  • Knowledge Of Healthcare Systems: The Candidate Must Have A Thorough Understanding Of The Healthcare Industry, Including Insurance, Medicare/Medicaid, And Managed Care Plans.

  • Strong Communication And Interpersonal Skills: As A Field-Based Case Manager, The Candidate Must Be Able To Effectively Communicate With Patients, Providers, And Other Healthcare Professionals. Excellent Interpersonal Skills Are Also Essential In Building Relationships And Collaborating With The Care Team.

  • Time Management And Organizational Skills: This Role Requires The Ability To Manage A High Caseload And Prioritize Tasks Effectively. The Candidate Must Be Able To Meet Deadlines, Multitask, And Maintain Accurate Documentation Of Patient Interactions And Interventions.
