- Provide personalized care management to members in their homes, utilizing clinical skills and expertise.

Registered Nurse (Rn) License: The Candidate Must Possess A Current And Active Rn License In The State Where The Job Is Located.
Case Management Experience: A Minimum Of 3-5 Years Of Experience In Case Management, Preferably In A Field-Based Setting, Is Required For This Role.
Knowledge Of Healthcare Systems: The Candidate Must Have A Thorough Understanding Of The Healthcare Industry, Including Insurance, Medicare/Medicaid, And Managed Care Plans.
Strong Communication And Interpersonal Skills: As A Field-Based Case Manager, The Candidate Must Be Able To Effectively Communicate With Patients, Providers, And Other Healthcare Professionals. Excellent Interpersonal Skills Are Also Essential In Building Relationships And Collaborating With The Care Team.
