Case Manager - Registered Nurse
The Nurse Case Manager is responsible for face to face assessing, planning, implementing and coordinating all case management activities with members to evaluate the care needs, psycho/social needs and medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action including a care plan to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Develop, implement, support, and promote Health Services strategies, tactics, policies, and programs that drive the delivery of quality healthcare to establish competitive business advantage for Aetna.
3+ years of clinical practice experience is required
Must be comfortable visiting members in their homes
Previous face to face case management experience is preferred
RN with current unrestricted license to practice in New Jersey is required
