  1. Provide personalized case management services to Aetna members in Macon, Piatt, or Champaign County, IL.
  2. Conduct thorough assessments of member needs and develop individualized care plans.
  3. Collaborate with healthcare providers and community resources to ensure effective and timely delivery of services.
  4. Monitor and track member progress and make necessary adjustments to care plans.
  5. Utilize analytical skills to identify opportunities for improvement and cost savings.
  6. Maintain accurate and detailed documentation of all case management activities.

Job Qualifications

  • Must Reside In Macon, Piatt, Or Champaign County, Il And Be Able To Travel Within The Designated Region For Field Work.
