  6. Conduct in-depth assessments of members' health and well-being to identify any potential risks or issues that require attention.
  7. Develop and implement care plans for members, taking into consideration their individual needs, preferences, and resources.
  8. Educate and support members in managing their health and navigating the healthcare system.
  9. Serve as a liaison between members, healthcare providers, and insurance companies to facilitate efficient and effective communication.
  10. Stay up-to-date on industry trends and advancements in healthcare to continuously improve care management processes and strategies.

Job Qualifications
  • Bachelor's Degree In Nursing Or Social Work, Or Related Field.
