- Coordinate care and resources for members enrolled in the OhioRISE program to ensure the best possible outcomes.
 - Work closely with a team of healthcare professionals to develop and implement individualized care plans for members.
 - Communicate effectively with members, caregivers, and healthcare providers to ensure coordinated and comprehensive care.
 - Advocate for the needs and rights of members, ensuring they receive appropriate and timely care.
 - Conduct comprehensive assessments of members' health and behavioral health needs.
 - Collaborate with community resources to address social determinants of health for members.
 - Monitor and track member progress, identifying potential barriers to care and implementing solutions.
 - Maintain accurate and up-to-date documentation of member interactions and care plans.
 - Participate in case conferences and team meetings to discuss member needs and treatment plans.
 - Work with members and their families to educate and empower them to actively participate in their own care.
 - Comply with all regulatory and accreditation standards, as well as company policies and procedures.
 - Continuously strive to improve the quality and efficiency of care for members.
 - Support the mission and values of Aetna by providing compassionate and person-centered care.
 - Stay updated on industry trends and best practices to enhance knowledge and skills.
 - Collaborate with other departments within Aetna to ensure seamless and coordinated care for members.
 - Adhere to all confidentiality and privacy policies to protect member information.
 - Attend trainings and meetings to enhance knowledge and skills related to the OhioRISE program.
 - Maintain a positive and professional attitude when interacting with members, caregivers, and colleagues.
 - Meet or exceed performance metrics and goals set by Aetna.
 - Represent Aetna in a positive manner and promote the company's mission and values.
 
Excellent Communication And Interpersonal Skills
Bachelor's Degree In Nursing, Social Work, Public Health, Or A Related Field
Minimum Of 2 Years Experience In Healthcare, Care Management, Or Related Field
Knowledge Of Ohio's Healthcare System And Resources
Strong Organizational And Time Management Abilities To Effectively Manage And Prioritize A Caseload Of Members
