As a Care Management Associate, you will play a crucial role in coordinating care and resources for our members, working closely with our team of healthcare professionals to ensure the best possible outcomes.   1. Coordinate care and resources for members enrolled in the OhioRISE program to ensure the best possible outcomes.
  2. Work closely with a team of healthcare professionals to develop and implement individualized care plans for members.
  3. Communicate effectively with members, caregivers, and healthcare providers to ensure coordinated and comprehensive care.
  4. Advocate for the needs and rights of members, ensuring they receive appropriate and timely care.
  5. Conduct comprehensive assessments of members' health and behavioral health needs.
  6. Collaborate with community resources to address social determinants of health for members.
  7. Monitor and track member progress, identifying potential barriers to care and implementing solutions.
  8. Maintain accurate and up-to-date documentation of member interactions and care plans.
  9. Participate in case conferences and team meetings to discuss member needs and treatment plans.
  10. Work with members and their families to educate and empower them to actively participate in their own care.
  11. Comply with all regulatory and accreditation standards, as well as company policies and procedures.
  12. Continuously strive to improve the quality and efficiency of care for members.
  13. Support the mission and values of Aetna by providing compassionate and person-centered care.
  14. Stay updated on industry trends and best practices to enhance knowledge and skills.
  15. Collaborate with other departments within Aetna to ensure seamless and coordinated care for members.
  16. Adhere to all confidentiality and privacy policies to protect member information.
  17. Attend trainings and meetings to enhance knowledge and skills related to the OhioRISE program.
  18. Maintain a positive and professional attitude when interacting with members, caregivers, and colleagues.
  19. Meet or exceed performance metrics and goals set by Aetna.
  20. Represent Aetna in a positive manner and promote the company's mission and values.

Job Qualifications
  • Excellent Communication And Interpersonal Skills

  • Bachelor's Degree In Nursing, Social Work, Public Health, Or A Related Field
