- Provide care management support to Aetna members to improve healthcare outcomes.
- Collaborate with a team of healthcare professionals to develop and implement care plans for members.
- Communicate effectively with members, providers, and other team members to ensure coordinated and quality care.
- Maintain accurate and up-to-date member records and documentation.
- Assist with scheduling appointments, coordinating referrals, and arranging transportation for members when necessary.
- Conduct outreach and engagement activities to identify and engage members in need of care management services.
- Monitor and track member progress and provide necessary follow-up to ensure goals are met.
- Participate in team meetings and contribute ideas for improving processes and outcomes.
- Adhere to all company policies and procedures, as well as regulatory guidelines.
- Continuously seek opportunities for professional growth and development to enhance job performance.
- Maintain confidentiality and handle sensitive member information with utmost care.
- Demonstrate empathy, compassion, and a non-judgmental attitude towards members.
- Collaborate with community resources to ensure members have access to necessary support services.
- Stay informed about industry trends and developments related to care management.
- Act as an advocate for members and work to address any barriers to their healthcare needs.
Bachelor's Degree In Healthcare Administration, Nursing, Social Work, Or Related Field.
Minimum Of 2 Years Experience In Care Management, Case Management, Or Healthcare Coordination.
Knowledge Of Medicare And Medicaid Regulations And Guidelines.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate And Build Relationships With Healthcare Providers And Members.
Familiarity With Electronic Medical Records And Proficiency In Microsoft Office.
