  1. Provide compassionate and personalized care management services to Aetna members.
  2. Collaborate with healthcare professionals to coordinate and support the delivery of quality healthcare services.
  3. Build strong relationships with members, understanding their unique needs and advocating for their best interests.
  4. Communicate effectively with members, healthcare providers, and other team members to ensure efficient and effective care coordination.
  5. Utilize clinical knowledge and resources to assess member needs and develop individualized care plans.
  6. Monitor and track member progress, making necessary adjustments to the care plan as needed.
  7. Coordinate and schedule appointments, referrals, and other healthcare services for members.
  8. Educate members on their healthcare options and available resources.
  9. Document all interactions and interventions accurately and in a timely manner.
  10. Stay up-to-date on industry trends, regulations, and best practices in care management.
  11. Participate in ongoing training and development opportunities to enhance skills and knowledge.
  12. Adhere to all company policies and procedures, as well as ethical and legal standards.
  13. Maintain confidentiality of member information and adhere to HIPAA regulations.
  14. Support and promote a positive and inclusive work environment.
  15. Take on additional responsibilities and projects as assigned by management.

Job Qualifications
  • Bachelor's Degree In A Related Field Such As Healthcare Management, Social Work, Or Nursing.

  • Minimum Of 2 Years Of Experience In Care Management Or Healthcare Coordination.

  • Knowledge Of Healthcare Systems And Insurance Processes.

  • Strong Communication And Interpersonal Skills.

  • Ability To Work Independently And Manage Multiple Tasks In A Fast-Paced Environment.
