- Coordinate and schedule appointments, referrals, and other healthcare services for members.
- Educate members on their healthcare options and available resources.
- Document all interactions and interventions accurately and in a timely manner.
- Stay up-to-date on industry trends, regulations, and best practices in care management.
- Participate in ongoing training and development opportunities to enhance skills and knowledge.
- Adhere to all company policies and procedures, as well as ethical and legal standards.
- Maintain confidentiality of member information and adhere to HIPAA regulations.
- Support and promote a positive and inclusive work environment.
- Take on additional responsibilities and projects as assigned by management.
Bachelor's Degree In A Related Field Such As Healthcare Management, Social Work, Or Nursing.
Minimum Of 2 Years Of Experience In Care Management Or Healthcare Coordination.
Knowledge Of Healthcare Systems And Insurance Processes.
Strong Communication And Interpersonal Skills.
Ability To Work Independently And Manage Multiple Tasks In A Fast-Paced Environment.
