  4. Communicate and coordinate with members, providers, and other stakeholders to ensure timely and effective care coordination.
  5. Maintain accurate and detailed records of member interactions and progress.
  6. Monitor and evaluate the effectiveness of care plans and make necessary adjustments.
  7. Stay up-to-date on industry trends and regulations to ensure compliance and provide informed recommendations.
  8. Work closely with the team to identify and implement best practices for case management.
  9. Provide support and guidance to other team members as needed.
  10. Demonstrate a strong commitment to providing exceptional customer service and patient care.
  11. Adhere to all company policies, procedures, and ethical standards.
  12. Maintain confidentiality and handle sensitive information with discretion.

Job Qualifications
  • Minimum Of 2 Years Experience In Case Management, Preferably In A Healthcare Setting.

  • Bachelor's Degree In A Related Field Such As Healthcare Administration, Social Work, Or Business.

  • Strong Analytical And Problem-Solving Skills, With The Ability To Interpret Data And Make Informed Decisions.

  • Excellent Communication And Interpersonal Skills, With The Ability To Work With Diverse Populations And Collaborate With A Multidisciplinary Team.

  • Must Reside In The North Or West Side Of Chicago, Il And Be Willing To Travel Within The Designated Area For Field Work.
