Job Description

In this role, you will play a critical role in ensuring the best possible outcomes for our members by utilizing your strong analytical skills and knowledge of healthcare processes.   1. Conduct detailed analysis of healthcare data to identify trends and patterns that can inform case management strategies.
  2. Utilize knowledge of healthcare processes to develop and implement effective case management plans for members.
  3. Collaborate with other members of the case management team to ensure coordinated and comprehensive care for members.
  4. Maintain accurate and up-to-date records of member cases and progress.
  5. Communicate and collaborate with healthcare providers and other stakeholders to ensure the best possible outcomes for members.
  6. Proactively identify and address potential issues or barriers to care for members.
  7. Stay up-to-date on industry trends and best practices in case management to continuously improve processes and outcomes.
  8. Adhere to all regulatory and compliance requirements in case management practices.
  9. Serve as a subject matter expert on case management processes and procedures within the organization.
  10. Participate in training and development opportunities to enhance skills and knowledge in case management.
  11. Act as a resource for colleagues, providing guidance and support as needed.
  12. Maintain a high level of professionalism and confidentiality in all interactions with members and colleagues.
  13. Contribute to a positive and collaborative team environment.
  14. Continuously seek opportunities to improve processes and efficiency within the case management department.
  15. Represent Aetna in a positive and professional manner in all interactions with external stakeholders.

Job Qualifications
