1. Provide compassionate care to patients and their families in accordance with AdventHealth's mission and values.
2. Collaborate with interdisciplinary team members to assess and address the social and emotional needs of patients.
3. Conduct thorough assessments to identify any barriers to care and develop individualized care plans.
4. Offer emotional support and counseling to patients and their families during difficult times.
5. Connect patients and families with relevant community resources and support services.
6. Advocate for patients' rights and assist with navigating the healthcare system.
7. Document all interactions and interventions in patients' medical records.
8. Maintain confidentiality and adhere to all ethical standards and guidelines.
9. Continuously evaluate and modify care plans based on patients' progress and needs.
10. Participate in team meetings, trainings, and other professional development opportunities.
11. Adhere to all state and federal regulations and laws related to social work practice.
12. Collaborate with insurance providers, government agencies, and other organizations as needed.
13. Serve as a liaison between patients, families, and healthcare providers.
14. Maintain accurate and up-to-date records and reports.
15. Provide crisis intervention and support to patients and families in emergency situations.
16. Stay informed about current trends and best practices in social work and healthcare.
17. Perform any other duties as assigned by the supervisor.
