Social Worker, ED Behavioral Health Program

1. Provide compassionate and holistic care to patients in crisis who are seeking mental health treatment in the emergency department.
2. Assess patients' mental health needs and develop individualized treatment plans.
3. Collaborate with interdisciplinary team members to provide comprehensive care and support to patients.
4. Utilize evidence-based practices to promote healing and wellness in patients.
5. Provide crisis intervention and de-escalation techniques as needed.
6. Conduct thorough psychosocial assessments and make appropriate referrals for follow-up care.
7. Advocate for patients' rights and ensure their needs are met.
8. Maintain accurate and timely documentation of patient interactions and treatment plans.
9. Stay up-to-date on current trends and developments in the field of social work and behavioral health.
10. Collaborate with community resources to provide patients with necessary support and resources.
11. Participate in team meetings and case conferences to discuss patient progress and develop treatment plans.
12. Provide education and support to patients and their families regarding mental health and available resources.
13. Adhere to ethical and legal standards of practice.
14. Ensure compliance with all hospital policies and procedures.
15. Continuously strive to improve the quality of care provided to patients.
16. Maintain confidentiality of patient information.
17. Communicate effectively with patients, families, and team members.
18. Act as a positive and professional representative of AdventHealth at all times.
19. Attend trainings and conferences to further develop skills and knowledge related to social work and behavioral health.
20. Perform any other duties as assigned by supervisor.
