Job Description

If so, we are seeking a highly motivated and compassionate Social Worker to join our Care Management team at AdventHealth.As a Social Worker, you will play a crucial role in providing comprehensive care management services to our patients and their families. You will collaborate with interdisciplinary teams and community partners to ensure that our patients receive the best possible care and support. Your empathy, advocacy skills, and knowledge of social work principles and resources will be essential in helping our patients navigate through challenging situations.We are looking for candidates who possess a Bachelor's or Master's degree in Social Work and have a minimum of 2 years of experience in a healthcare setting. Candidates must also have a current state license or be eligible for licensure as a Social Worker.   1. Provide comprehensive care management services to patients and their families.
  2. Collaborate with interdisciplinary teams and community partners to ensure patients receive the best possible care and support.

  4. Utilize empathy and knowledge of social work principles to assist patients in navigating challenging situations.
  5. Conduct assessments and develop individualized care plans for patients.
  6. Monitor and evaluate patient progress and make necessary adjustments to care plans.
  7. Maintain accurate and timely documentation of patient interactions and interventions.
  8. Educate patients and their families on available resources and support services.
  9. Stay up-to-date on current social work practices and resources.
  10. Adhere to ethical and legal standards of practice.
  11. Collaborate with healthcare providers to address social and emotional needs of patients.
  12. Participate in team meetings and provide input on patient care.
  13. Maintain confidentiality of patient information.
  14. Advocate for social justice and promote social change.
  15. Attend trainings and conferences to enhance knowledge and skills.
  16. Comply with all organizational policies and procedures.
  17. Fulfill all state and federal requirements for social work licensure.
  18. Demonstrate a commitment to providing high-quality and compassionate care to patients.
  19. Participate in quality improvement initiatives.
  20. Support the mission and values of the organization.

Job Qualifications
  • Bachelor's Or Master's Degree In Social Work From An Accredited University

  • Minimum Of 2 Years Experience In Care Management Or Social Work, Preferably In A Healthcare Setting

  • State Licensure As A Social Worker (Lcsw Or Lmsw)

  • Knowledge Of And Experience With Case Management Principles And Techniques
