1. Conduct comprehensive psychosocial assessments for patients to identify their social and emotional needs.
2. Collaborate with interdisciplinary teams to develop effective care plans for patients, taking into consideration their social and emotional needs.
3. Provide counseling and support to patients and their families, addressing any emotional or psychological issues that may impact their health.
4. Connect patients and their families with community resources and support services to ensure continuity of care.
5. Advocate for patients and their families to ensure their needs are being met and their rights are being respected.
6. Maintain accurate and detailed documentation of patient interactions and progress in their care.

11. Participate in team meetings and provide input on how to improve patient care and outcomes.
12. Collaborate with external agencies and organizations to provide comprehensive care to patients.
13. Maintain confidentiality and adhere to ethical standards, HIPAA regulations, and other legal requirements.
14. Participate in ongoing education and training to stay current in the field of social work.
